The biography information will be shared with others attending the Immersion Excursion. All other information will remain personal and confidential and viewed only by Church Doctor Ministries staff. I am aware that I may be required to meet all entry requirements for travel to the UK and to the US. I am aware that fees associated for entering the UK are not covered by my trip fees paid to Church Doctor Ministries. (initial below)*
I am aware Church Doctor Ministries has no authority or control over trip/travel restrictions and changes made to the trip agenda due to pandemic-related issues or concerns. Decisions and restrictions made by international governments, travel, transportation, hotel companies, host sites, or any quarantine requests or guidelines made during the trip will strictly be followed. I understand all trip fees paid to Church Doctor Ministries are nonrefundable. If I am unable to participate in the trip for any reason, I may find a replacement traveler from my church or attend a future England Immersion trip. Replacement travelers must complete the application and be approved by Church Doctor Ministries. I understand these fees do not cover vaccines, testing, or any costs associated with quarantine or medical care needed during my travel. (initial below)*
I understand that should I fall ill, become sick for any reason, or experience any type of injury, my travel team will be responsible to assist with my care. I understand that members of the Church Doctor Ministries team are not responsible for my care in this situation. (initial below)*
